REGISTRATION FORM
AGES: The DDC Gym-N-Learn Program is for children ages 3 - 5 years. Exceptions will be made on an individual basis.

DAYS & TIMES:

Wednesday and Friday, 10:00am - 12:00pm


GYM ATTIRE: Girls: gymnastics leotard with a pair of shorts (we can order); Boys: T-shirt & shorts; A sweatshirt should be brought for cooler weather

CURRICULUM: (Weekly themes will be used)
Our program focuses on:
Language - letters and sounds, word recognition, reading readiness, and learning with music and songs
Math - counting, recognizing numbers and learning through number games and activities

Science - how things look, feel, work, smell, and sound
Fine Motor Skills - working with puzzles, coloring, cutting, tracing, printing and writing the alphabet
Foreign Language - simple phrases, words, colors, and numbers
Art - creative expression, working with colors, shapes, and textures
Gymnastics - Gymnastics positions, various skills using uneven bars, balance beam, trampoline, and tumble track (at least 1 hour of gymnastics each day.

PRICES:
2 hrs/week $135/month
4 hrs/week $230/month
Additional Siblings: 2 hrs/week $115/month; 4 hrs/week $210/month
Supply Fees: $35 due at registration; $25 due February 1, 2012

Other Fees:

-Non-refundable registration fee of $45, or $85 for 2 or more siblings

-A $25 late fee will be charged for tuition payments received after the 5th of the month

-A $30 fee will be charged for all returned checks

Tuition is based on the year from September – May
